Privacy Policy

The short version

We collect your email address if you choose to give it to us. That is the only personal information we store.

We do not store your quiz answers. We do not run analytics, advertising pixels, or cookies. We do not sell or rent your information to anyone.

What we collect

On the website, when you ask us to email your plan, request to be matched with a provider, or download a guide, we store:

• Your email address.
• Which of those three things you asked for, so we send you the right follow-up.
• Where you arrived from, if that was included in the link you followed.
• The date and time you submitted it.

What we deliberately do not collect

Your answers to the questionnaire are never sent to us or saved on our servers. Your plan is generated from your answers and encoded directly into your plan link, which means the link itself carries the information and we hold no copy of it.

A practical consequence worth understanding: anyone who has your plan link can open your plan. Treat it like a private link and do not post it publicly.

We do not ask for your name, address, phone number, date of birth, or any medical records, and we have no way to connect an email address to a set of answers.

The Prezerv app

The plan you build and anything you log is stored on your device. The app works completely offline and sends nothing on its own.

There is exactly one thing that leaves your phone, and only if you tap it: choosing to have your plan emailed to you sends your email address, exactly as the website does. Your answers are not sent as data to be stored; they travel encoded inside the plan link so the email can open your plan, and we keep no copy.

Reminders, such as a prompt that a retest is due, are scheduled locally on your device. Setting one sends nothing to us or to anyone else.

Deleting the app deletes what was stored on the device. Because it never left your phone, we cannot restore it for you.

Who else touches your data

We use two service providers, and only for the purposes below. Neither is permitted to use your information for their own purposes.

• Supabase: hosts the database that stores the email addresses described above.
• Resend: sends the welcome email after you sign up.

How long we keep it

We keep your email address until you ask us to delete it, or until we shut the project down, whichever comes first.

Ask and we will delete it. We do not require a reason.

Your rights

If you are in the United Kingdom or the European Economic Area, you have the right to ask for a copy of what we hold about you, to have it corrected, to have it deleted, and to object to how we use it. Since what we hold is an email address, all of those requests are quick to satisfy.

Email hello@prezerv.co and we will action it. You also have the right to complain to your local data protection authority; in the UK that is the Information Commissioner's Office.

We rely on your consent as the legal basis for sending you email, and you can withdraw it at any time.

Children

Prezerv is intended for adults. It is not directed at anyone under 18 and we do not knowingly collect information from children.

What we are not

Prezerv is a planning and education tool. It does not diagnose, treat, or prescribe, and it is not a substitute for advice from a qualified clinician. It is not a regulated medical device.

We are not a healthcare provider, so we are not a HIPAA covered entity, and we make no claim to any security certification we have not actually obtained.

Changes

If we change what we collect, we will update this page and change the date at the top. Material changes will also go to anyone on the email list.
